Updates in Hepatitis B-related Liver Cancer Care
Image courtesy of stockimages at FreeDigitalPhotos.net While liver cancer cases continue to climb in the U.S., so has the medical community’s ability to care for hepatitis B patients affected by liver cancer. Here are some updates and reminders to help you talk to your doctor and get the best care possible. What causes your cancer matters: Doctors often use a one-size-fits-all approach to treating liver cancer, no matter if it’s caused by hepatitis B or C. However, a new study suggests doctors should consider treating their hepatitis B cancer patients earlier and more aggressively than hepatitis C patients. Researchers monitored liver cancer in hundreds of hepatitis B and C patients over nine years and found hepatitis B patients developed cancer at a younger age and had more aggressive cancer. Hepatitis B patients: Had higher levels of alpha-fetoprotein (AFP), a blood test that indicates the presence of tumors Had liver cancer tumors that were larger and spread more quickly And often had portal thrombosis–a blockage in the vessel that delivers blood to the liver. Antivirals help prevent cancer recurrence after surgery and transplants: Treating liver cancer patients with antivirals after their tumors have been removed or after liver transplantation clearly helps hepatitis B patients live longer. A growing number of studies show that treating transplant patients with antiviral drugs, which stop the virus from replicating, is improving survival because it stops the infection from returning. Treating patients with antivirals after their tumors are surgically removed or through chemoembolization, which blocks the blood supply to tumors, also extends patients’ lives. Treating depression is vital: Depression is very common in cancer patients and studies show 75 percent of cancer patients have untreated depression. The average survival rate for hepatitis B patients was 9.3 months after liver cancer diagnosis, compared to 10.9-month survival for hepatitis C patients. According to a related editorial published in the medical journal Oncology, about 700,000 patients are diagnosed with liver cancer each year worldwide; more than two-thirds of new cases result from HBV infection. Knowing HBV-related liver cancer is more aggressive can lead to different or accelerated treatment plans when patients are first diagnosed, the study's experts suggest. “This study provides more evidence that future clinical trials should stratify patients by hepatitis type to help identify better drugs and create personalized treatment modalities,” researcher Ahmed Kaseb explained. Kukka Cholesterol and Type 2 Diabetes Drugs Markedly Lower Cancer Risk in Hepatitis B Patients A large, ground-breaking study that followed 71,824 patients infected with the hepatitis B virus (HBV) from 2000 to 2008 found that those who took medium to high doses of metformin and/or statins had substantially lower rates of liver and other cancers. The study, conducted by Taiwanese researchers and published in the February issue of the journal Medicine, examined the impact of: Statins: Drugs taken to reduce triglycerides and cholesterol in the blood And metformin: A drug used to treat patients in the early stages of type 2 diabetes. There have been some studies that suggest statins reduce liver cancer risk in hepatitis B patients, however this large study that spanned eight years found a definite decrease in cancers in patients who took these drugs, especially at higher doses. Kukka Quality of Care for Women with Hepatitis B Varies Dramatically Across U.S. A U.S. Centers for Disease Control and Prevention study, published in the journal Infectious Diseases in Obstetrics and Gynecology, found a large discrepancy in medical treatment for women with hepatitis B-related liver disease. While scientific evidence shows antivirals are safe in pregnant women and effectively lower the risk of mother-to-child infection, American doctors prescribed antivirals to only 12.6% of pregnant women infected with the hepatitis B virus (HBV). In contrast, they prescribed antivirals to 20% of non-pregnant women. The important study examined prescription records of 17 million, privately-insured women in 2011. Arrowhead’s HBV Candidate Requires Further Dose Escalation
It should be noted while numerically the improvement in knockdown from 1mg/kg to 2mg/kg was only 12%, this is likely the result of the apparent high variability at the lower dose level with the increased tightness of the knockdown range at 2mg/kg indicating that the RNAi mechanism is starting to be solidly engaged with the expectation of a steepening dose response going forward. Sadly, we read that Phase 2a data presented by Arrowhead fell short of expectations for their ARC-520 drug to treat chronic hepatitis B. Hopefully dose escalation to 4mg/kg will result in both effective and safe results. However, there are others in the race for the cure, and may the most effective and safe drug soon result in a functional cure for chronic HBV. ~ hepbtalk Harnessing the Power of RNAi Gene Silencing in Quest of a Cure for Chronic Hepatitis B, and the HBV KnockDown blog written by Dirk Haussecker, who believes it's about time everyone got serious about a functional cure for hepatitis B. Be sure to visit Dirk Haussecker's blog ! Today, we learned about some hard HBsAg knockdown numbers from the phase IIa Hong Kong study of ARC520 in chronically infected HBV patients. The data relate to the first 2 cohorts in this ongoing dose escalation trial. Accordingly, the mean HBsAg knockdown at nadir for the starting dose of 1mg/kg was 39% within a range of 22-57%(n=6) while it was 51% within a range of 46-59% for the 2mg/kg cohort (n=6). ARC520 was given as a single dose to patients already stably on polymerase inhibitor entecavir. While clearly missing the company’s own guidance of a 1 log reduction at 2mg/kg, the good safety profile-no SAEs at all in the study with all AEs rated to be unrelated to ARC520- in addition to the steepening dose-response curve following 2mg/kg means that ARC520 is far from being out of the HBV knockdown race. Talking with Your Doctor About Clinical Trials
It is important to make an informed decision when considering a clinical trial, and talking to your doctor should be part of this process. Bring the information about any trials with you when you visit your doctor, so you can discuss the pros and cons of each. Your doctor should be able to help explain the process of joining a clinical trial, and you can make a shared decision about what is best for you. Remember that when you join a clinical trial, you will be working with the health care team that is associated with the trial – this might mean seeing a different doctor than you do now. Be sure to coordinate your care among your doctors. Your Doctor's Role in Clinical Trials Talk to your doctor about clinical trials that may benefit you. Ask your doctor to help you sort through the pros and cons of enrolling in a clinical trial and the different treatment options. Be sure to ask about whether the drug being tested is better or riskier for you than no treatment. Is it better than existing treatments that are already approved? Although you can leave a clinical trial at any time, be sure to let the doctor or nurse know why you want to leave. You are also encouraged to see your primary health care provider on a regular basis even if you are in a clinical trial. 10 Questions to Ask Your Doctor Before Enrolling in a Clinical Trial Where is the site located and how often will I need to go there? Will I be charged for any part of the trial or will my insurance pay for it? Why do researchers think the treatment will work better than standard (FDA-approved) treatment? Who is most likely to benefit from this drug? How many people have tested the drug and what has happened to them? What are the possible short and long-term side effects? What tests or procedures are involved and how are patients safely monitored while on the treatment? What happens to my hepatitis B when I stop taking the drug? What is the follow-up care after the clinical trial ends? If this drug doesn't work for me, am I eligible to try another one?
